Health Forms

In accordance with the New York State Public Health Law, ALL students attending New York City Public Schools must be immunized against the following diseases: diphtheria, pertussis, tetanus, polio, measles, mumps, rubella, varicella, Hepatitis B and Haemophilus influenzae type B (Hib). Immunization records are evaluated according to a child’s birthdate – as new shots are given, your child’s immunization record will be updated in the DOE database.  All children must have proof of immunization before they can be registered for school. In addition, every student must have a health form, signed by a physician, on file in school. A family may apply to the DOE for a waiver from immunizations for medical or religious reasons.
